This committee meets as needed on the third Wednesday of the month, we cannot process forms until all documentation has been provided. The committee will review the document and then contact the PI and Thesis Adviser (if applicable) by email about the second step. This will either be a request for clarification/additional information or will be to let you know that the form has gone to the next level. Once the document has made it through all levels, you will be notified via email of approval. All hard copy requests will be stored with the College committee while electronic copies will be stored with the University committee. Please remember student researchers must complete the following training: https://phrp.nihtraining.com/users/login.php, and provide the completion certificate to the committee with the forms.
