The University provides full-time, regular and term, faculty and staff members with basic life insurance equal to $25,000 of coverage. This coverage is term coverage and ends upon termination of employment. Coverage becomes effective on your date of hire.
In addition, at the time of hire, all eligible participants may elect 1x, 2x, 3x, 4x, or 5x base salary coverage rounded to the next higher multiple of $1,000. The maximum amount of coverage available is $750,000. The guaranteed issue limit is $100,000. If you elect coverage that exceeds the guaranteed issue amount of $100,000, you will need to complete the Standard's Medical History Statement providing evidence of good health that is satisfactory to the Standard Insurance Company before the excess can become effective.
If you waived Supplemental Life Insurance when initially eligible and wish to purchase coverage later, medical evidence of insurability will be required. If you currently have coverage and wish to increase the amount of coverage, medical evidence of insurability will be required for only the increased amounts.
Benefits are reduced to 65% at age 65, to 42% at age 70, to 27% at age 75, to 20% at age 80, and 15% at age 85, to 12% at age 90, and to 9% at age 95.
At the time of hire, dependent life insurance can be purchased in amounts of $5,000, $10,000 and $15,000 coverage plans. Your eligible dependents include your spouse and your dependent children. A child’s coverage will end on the first day of the calendar year following the child’s 26th birthday. You become eligible to insure your dependents on the later of a) the date your life insurance becomes effective; and b) the date you first acquire a dependent. A participant may not be insured as both an employee and a dependent. A child may not be insured by more than one employee.
Accidental Death and Dismemberment
The University also provides Accidental Death and Dismemberment coverage to all eligible employees equal to $25,000. ADD provides a benefit payment to members, or their families, when certain losses occur within 365 days of an accident.
The University reserves the right to modify or terminate such plans at any time with or without notice. Participation in these plans is provided to eligible employees and does not constitute a guarantee of employment. Participation is subject to the terms and conditions specified in the plan documents.
