Gift rewards reminder

Incentives can be earned only once every 365 days. For example, if you complete a Health Assessment today and receive your $75 reward, you need to wait 365 days from today to complete another Health Assessment to receive an additional $75 reward. This also pertains to the online and telephonic coaching rewards.

2. Click the Wellness tab
3. Click the Rewards Icon
4. See date last rewards was posted on
5. If date is longer than 365 days please take the Health Assessment
6. If date hasn’t passed 365 days please wait until the 366th day to take your next Health Assessment.

New Urology In-Network Provider

We are pleased to announce that effective January 1, 2012, Cape Girardeau Urology Associates is an in-network provider for UnitedHealthcare. For complete details about this health care provider, click on the "Find a Doctor" search feature button on the UnitedHealthcare website.

Explanation of Benefits Document Redesigned with Enhanced Features

The revised Explanation of Benefits (EOB) helps members more readily understand what their health plan has paid on claims and how much they may owe their provider. View the EOB flier for more information and examples of the revised EOB.

Prescription Drug List and Pharmacy Benefit Updates for July 1, 2012

Prescription Drug List (PDL) and benefit coverage updates will take place on our Advantage Prescription Drug List on July 1, 2012. The following links provide you with general benefits information, forms, enrollment information, and links to University providers and other benefit resource sites. Benefits coverage is effective on your date of hire or benefit eligibility date. Enrollment must be completed within 31 days of your date of employment or benefit eligibility date. After the initial enrollment, changes to your enrollment will only be allowed during the University's annual open enrollment period unless you have a qualified family status change.
