Graduate Assistant - Approval/ Reappointment Form

Use this form when:

·        A new Graduate Assistants is hired

·        A Graduate Assistant is going to continue to work the next Academic Year

 

Graduate Assistant - Approval to Work Additional On-Campus Job Form

Use this form when:

·        A Graduate Assistant will be working during Winter/ Summer Break

·        To obtain approval from Graduate Studies Department

 

Graduate Assistant - Evaluation Form

Use this form when:

·        To Evaluate Graduate Assistants

·        Submit this form each semester before the week of finals

 
Graduate Assistant - GA Annual Evaluation of Assistantship

Use this form when:

·       To evaluate the Assistantship

·       Graduate Assistants will complet this form every year after working as a GA 

 
Graduate Assistant - GA Checkout Form

Use this form when:

·      Graduate Assistant contract expires

·      And This is a mandatory form needed to receive full clearance from the University
