Wellness Screening

Southeast Missouri State University is committed to helping students, faculty and staff and their spouses to maintain a healthy lifestyle by providing beneficial fitness and wellness opportunities. The Annual Wellness Screening provided by the Department of Recreation Services isFebruary 20 - 24, 2012 from 6 to 10 a.m. in the Student Recreation Center - North, Room 201. Prior registration is necessary.

University employees, spouses, and retirees who are enrolled in the University health insurance plan coverage may participate in the wellness screening with no out-of-pocket cost for basic services. All claims will be filed with the employee’s health insurance. Individuals may opt out and pay the cost out-of-pocket as well. Optional services will be also available with nominal fees.

Students, spouses and retirees who do not have the University health insurance plan coverage or who have Medicare as their primary insurance may participate in the wellness screening with nominal fees. A receipt indicating what services are paid for by a participant will be provided for insurance reimbursement purposes. Please contact your health insurance provider for their reimbursement process, if applicable.

Wellness Screening Components

For Southeast Employees & Retirees and Spouses with the University’s United Healthcare insurance plan (Retirees and their spouses should be younger than 65 and are NOT using Medicare as their primary to qualify):

The following services are offered free of charge

  • Blood Pressure
  • Height/Weight
  • Flexibility
  • Muscular Endurance
  • Vision
  • Hearing

The following services will be offered at no cost and will be filed with the insurance plan:

  • Basic Blood Analysis – lipid panel/full cholesterol panel, glucose,: 10-hour fast required for accurate blood analysis
  • Thyroid - TSH
  • Hemoglobin A1C
  • PSA - Prostate Specific Antigen (only if male over 40)
  • Heel bone density test
  • Flu shot (available on Feb. 22)

The following services will be offered with nominal fees;

  • Complete blood count (CBC) ($4.20)
  • Thyroid - T4 ($8.81)

For Southeast Employees & Students and Spouses WITHOUT the University’s United Healthcare insurance plan and Retirees and their spouses who are over 65 and using Medicare as their Medicare as primary.

The following services are offered free of charge:

  • Blood Pressure
  • Height/Weight
  • Flexibility
  • Muscular Endurance
  • Vision
  • Hearing

The following services will be offered with nominal fees

  • Basic Blood Analysis – lipid panel/full cholesterol panel, glucose,: 10-hour fast required for accurate blood analysis ($16.20)
  • Thyroid - TSH ($18.00)
  • Hemoglobin A1C ($17.23)
  • PSA - Prostate Specific Antigen (15.80)
  • Heel bone density test ($19.00)
  • Flu shot (available on Feb. 22) ($16.00)
  • Complete blood count (CBC) ($4.20)
  • Thyroid - T4 ($8.81)

Payment Options

All screening fees are due at the time of service. A billing statement will be mailed to you via campus mail or to your home prior to your screening appointment.

Payment methods include check, Money Order, Student Charge (Southeast students only), or University Charge (Southeast employees only). Exact payment is required. NO DEBIT/CREDIT CARD PAYMENT WILL BE ACCEPTED (Including a debit card from Medical Reimbursement Accounts and/or Health Savings Accounts).

*We cannot accept a debit card from Medical Reimbursement Accounts and/or Health Savings Accounts. A receipt indicating what services are paid for by a participant will be provided for insurance reimbursement purposes. Please contact your health insurance provider for their reimbursement process, if applicable.

Regristration

To participate in the Annual Wellness Screening, prior registration is required. The web-based registration system is now available by clicking the link below. This site is on a secure server and you will be asked to enter your Southeast ID number and PIN. Your PIN is a four-digit code made up of the day and year you were born, in the form of “ddyy” (i.e. October 2, 1970 = 0270).  
Southeast students and retirees/their spouses who have not participated recently need to contact Dr. Takeshi Fujii, Assistant Director, Fitness & Wellness at (573) 651-2367 to gain access to the online registration system.

Student Recreation Center - North:
Monday - Thursday: 5:30 a.m. - Midnight
Friday: 5:30 a.m. - 9 p.m.
Saturday: 10 a.m. - 9 p.m.
Sunday: Noon - Midnight

Student Aquatic Center:
Monday - Thursday: 5:30 - 7:30 a.m.; 11 a.m. - 9 p.m.
Friday: 5:30 - 7:30 a.m.; 11 a.m. - 8 p.m.
Saturday: Noon - 6 p.m.
Sunday: Noon - 8 p.m.

Student Recreation Center - South:
Monday - Thursday: 5:30 a.m. - 9 p.m.
Friday: 5:30 a.m. - 7 p.m.
Saturday: Closed
Sunday: 5 - 9 p.m.

All facilities will be closed: April 18 & 20. Hours will be reduced March 15-22 and April 19.
